Intensive Case Management

Intensive Case Management (ICM) is community-based care that provides mental health treatment support services and care coordination to improve the functioning, gaining access to necessary services, and creating an environment that promotes recovery.   

Our recovery approach assists individuals to identify, obtain and sustain a range of resources necessary to improve daily living, independent community functioning and personal development.

Services are provided in office, in the home or community setting.

Eligibility

Eligible individuals are diagnosed with a severe and persistent mental illness that interferes with their ability to live in the community, and one or more of the following:

  • Transitioning or discharged from psychiatric inpatient facility within the last 6 months.

  • Admitted to psychiatric inpatient facility or crisis stabilization unit 3 or more times in past 12 months

  • Continuously homeless for a year or more, or 4 episodes of homelessness in the past 3 years.

  • Released from jail or prison in the past 6 months

  • Seen in an emergency room for behavioral health needs 3 or more times in the past 12 months

  • Transitioning or recently discharged from ACT team.
