What Information Does CFI Collect About Users of the Site?

At the Site, we do not collect Personal Information from you unless you provide this information to us voluntarily and knowingly.  Users of the Site may complete the various forms outlined below for use or support of CFI’s services.  When users voluntarily fill out the forms below, they may provide CFI Personal Information, such as names, addresses, e-mail addresses, credit card details, or mental health medical information.

Volunteer Form:  If you would like to volunteer at CFI, then we may ask you to complete a volunteer form that can be found on our Site.  In the volunteer form, we ask for your name, email address, phone, street address, and any technical skills so that we may contact you for a volunteer opportunity that best fits your skill set.  We will store this information in our database for your current request and future volunteer opportunities.

Service Referral Form:  You may be interested in exploring CFI services for your use or that of a family member or friend.  For us to better evaluate this potential referral, and whether our services would be a good fit for this referral, we may ask that you complete the service referral form on our Site or you may download the form separately and send us a completed form via email or U.S. post office mail.  This form asks you to provide information about yourself or the person you are referring which may include: name, date of birth, social security number, telephone, employment (if any), income (if any), and some available mental health information.  We will also store this information in our database for the provision of our Services.

Ways to Give/Donations:  If you choose to donate through the Site, we may collect more information about you depending on how you choose to donate.  You may donate funds via mail, or electronically.  If you donate electronically, then you will be provided a link to a third-party site (powered, at this time, by Paypal).  Paypal will then request your credit card number, bank account information or other details.  Paypal will process your donation.  Paypal provides its own data privacy and security policies on its website.  Please read and review these documents to better understand how Paypal may use and share your Personal Information.

Contact Us:  You may choose to contact us by selecting the “Contact Us” tab.  When you click that tab, we ask for your name and email address so that we may be able to contact you.  We will hold that information in our database so we can contact you upon your request or at some point in the future.
